ColonoscopyAssist is the organization that will be scheduling your procedure. We will be responsible for :
1) Scheduling your procedure
2) Collecting any applicable payments
3) Providing you with a statement and a copy of your report (within 4 weeks)

Facility Selection :
You will be scheduled at No Facility Selected. To change the facility, please return to the facility map page and select an alternate center.

Time to appointment :
Appointments are scheduled based on your preference entered in the scheduling section. This facility scheduling about 14 days out for non urgent requests.
For urgent requests, please mark as urgent while entering scheduling preferences.

Payments :
This form is an appointment scheduling request only. No payment is needed at the time of completing forms.
The forms will let you know what your total cost of the procedure(s) will be. Payments are due 7 days prior to your scheduled appointment date.
If paying by check, please ensure the check arrives by the due date.
